Disaster Unemployment Assistance - DUA Information Pamphlet (DUA-7)
This pamphlet provides basic information explaining the objective of the Disaster Unemployment Assistance Program. It lists general eligibility requirements and instructions for initial claims filing if you have lost employment or had employment interrupted as a direct result of a presidential declared major disaster. The DUA Information Pamphlet also offers information about continued weekly claims filing and payment computation. It lists the toll free DUA benefits information number: 1-866-795-8877.

Voluntary Election for Withholding of Individual Income Tax and or Direct Deposit of UI Benefits  (NCUI 500TW)
Unemployment Insurance benefits are taxable for both Federal and State government. You may elect to have the Employment Security Commission withhold a portion of your benefits each week to meet this tax obligation. You may also elect to have your benefit check directly deposited into your bank account instead of being delivered by the US mail. To request either of these options, please click Here.

Work Search Record  (NCUI 506E)
The Employment Security Law requires claimants to actively seek work. Commission Regulation 10.25 defines the minimally acceptable work search effort as two in-person contacts with different employers on different days for each week claimed.

Information about Unemployment Insurance for Workers on Temporary Layoff  (NCUI 517x)
Your employer will notify you when a temporary layoff occurs or is pending, and will also inform you of the time and place where you will be required to furnish information for unemployment insurance benefits. Your employer is required to give you this form if you become temporarily unemployed.

The following information is a summary of the legal requirements and your rights and responsibilities while filing a claim for unemployment insurance benefits. Read all of this information carefully and refer to it as needed. If there is anything you do not understand, be sure to ask a member of the Employment Security Commission staff for an explanation. You may obtain a printed copy of this information by requesting a copy of the Claimant Information and Identification Booklet, Form NCUI 504 from your nearest office of the Employment Security Commission.

Pre-Examination Booklet for Employment Consultants
The Selection Test for Employment Consultant, Veterans’ Employment Consultant, and Disabled Veterans’ Employment Consultant is 2 hours long and consists of 105 multiple-choice questions that measure some of the abilities that one needs in order to be an employment consultant. The pre-examination booklet describes different areas of the test and provides sample questions for each area. It enables applicants to gain a comfort level with the examination before having to take the test. The Verbal Comprehension section provides a list of vocabulary words that appear in the test and are often used by employment consultants in their work.
